As a volunteer, I expressly assume the risk and legal liability related to my activity with Cultivate Kansas City. I waive and release all claims for injuries, damages or loss which I may sustain as a result of my participation. I understand that I am solely responsible for my own safety both on the premises and in traveling to and from the work sites.

I warrant that I have no physical or psychological medical conditions that would create risk(s) for others or myself in connection with this volunteer activity.

By submitting the following form, I hereby give my permission for Cultivate KC to use photographs or video footage taken during my time as a volunteer in publications, presentations, websites and/or other promotional materials. I gladly give this authorization to support the efforts of Cultivate KC, and I understand that this authorization shall continue until terminated in writing.

By executing this form and submitting it, I express I have read, understand and intend to enter into this Release and Waiver of Liability for myself or minor willingly and voluntarily.
