AGREEMENT OF RELEASE AND WAIVER OF LIABILITY/TERMS AND CONDITIONS(CLICK ALL BOXES to secure trial) *

Vadim Podosyan, The Canadian Centre For Capoeira, Groove Creative Arts Centre, Dance Connexion, their officers, directors, employees, volunteers, members and representatives are not responsible for any injury, loss or damage of any kind sustained by any person while participating in the Capoeira Classes, Capoeira “Rodas” (Circle), Fitness Classes, Programs, Workshops, Seminar, Ceremonies, Annual Events, Capoeira Performances and/or Demonstrations.

I, my heirs, or legal representative or such forever release, waive, discharge and covenant not to sue or litigate in any way, Vadim Podosyan, The Canadian Centre for Capoeira, Dance Connexion, Groove Creative Arts Centre, any Capoeira instructors or participants, the leaseholder or owner of the building for any injury or death caused by their negligence or other acts.

My child is participating in these Capoeira Classes, Capoeira “Rodas” (Circle) Fitness Classes, Programs, Workshops, Seminar, Retreats, Ceremonies, Annual Events, Capoeira Performances and/or Demonstrations during which I will receive information and instruction about Capoeira and/or Fitness. I recognize that participation in the activities listed above requires physical exertion, which may be strenuous and may cause physical injury and I am fully aware of the risks and hazards involved.

In consideration of being permitted to participate in Capoeira Classes, Capoeira “Rodas” (Circle), Fitness Classes, Programs, Workshops, Seminar, Retreats, Ceremonies, Annual Events, Capoeira Performances and/or Demonstration, I sign this agreement and agree to assume full responsibility for any risks, injuries and/or damages, known or unknown, which my child might incur as a result of participating in the program.

Media Use Consent: The Canadian Centre for Capoeira, and Vadim Podosyan, reserves the right to use and display any and all media (photo, film, etc.) obtained of its members for use in advertisement of their facilities, classes and/or events.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
