Please take a moment to fill out this information.  Any medications used by the student must be brought to the school nurse with a note from the doctor and in the original container (labeled).  Extra EpiPens and Inhalers must also be kept by the school nurse.  If you have any questions please email Lisamarie Berrios at lberrios@academyprep.org.
