ASPIRE & ACHIEVE: Alternative Provision for 11-16 Learners on Roll of Leeds’ Schools

This booking form is to be completed for the sole purpose of purchasing a place at Aspire and Achieve for 11-16 year old learners on the roll of Leeds’ schools and forms part of a Service Level Agreement between Aspire and Achieve and ‘Home School’.

It is agreed that in respect of each booking placed both parties shall abide by terms and conditions and any additional special terms that have been agreed.  

In completing this booking form, you ("the School") hereby confirms that it agrees with the Service Level Agreement.
