-- Print the form, sign it, and take it, along with the required documentation, to a school representative to obtain the required school representative's signature.
-- Then return this form (do not include the accompanying documentation) along with the summer camp registration form to the Carver Educational Service Center (CESC), 850 Hungerford Dr., Room 254, Rockville, MD 20850. Forms are accepted via MCPS pony mail (from your child's school), by hand delivery, and by fax to 301-279-3340. Cash payment requires hand delivery.

-- Acknowledgement of the receipt of your form will be sent via e-mail.
