MIND Mentor Application 2026 - 2027
If you are interested in becoming a MIND Mentor for the 2026 - 2027 school year, please complete the questions below by September 1st. Mentors should review the MIND Mentor Information (visit https://parkcityreads.org/mind/) and be in grades 9-12. 

Mentors will be expected to attend at least 4 sessions of our afterschool program for middle school students that will be held on the following Wednesdays from 3:45 - 5:00 pm: 10/7, 11/4, 12/2, 1/13, 2/10, 3/10, 4/7 and 5/5.

Past Mentors do not need to reapply.

Acceptance into the Mentor Program is dependent on space available and previous participation in MIND programs is taken into consideration.
