IN-FORM1 (FY 2024-25)

IN-FORM1 must be submitted by June 15 each fraternal year (FY) even if there are no changes from the previous fraternal year or some information is not yet known.

Many questions require a response.  If the information for a required response is unknown (e.g., Gibault Envoy has not yet been assigned) or does not exist (e.g., council has no website), please enter the supplied response noted in that question's Description.

Please do not use all uppercase/caps for any answers.  Please do not use uppercase/caps in email addresses.

PDF versions of Supreme Forms 185 and 365 should be emailed to the State Council when the IN-FORM1 is submitted (send using separate email and do not include Forms 185 and 365 should be emailed to  Do not complete additional forms – send copies as submitted to Supreme.

The online version of IN-FORM1 is automatically sent to the State Administrative Assistant when submitted - do not send it again via email.  Please submit a revised IN-FORM1 whenever new information becomes available or any previously-submitted information changes.
