Please complete this form to sign up for 4 weeks of Backyard Games/Outdoor Choices through the Linwood Athletics Club with parent or guardian.

The dates of Backyard Games/Outdoor Choices club will be the following Tuesday's:

Please note, due to capacity, only the first 20 5th graders and 20 6th graders will be selected.

If you will be riding the late bus, please look at the late bus routes with a parent/guardian to determine which bus you will ride.

You will be notified by the club advisor upon acceptance by Google Classroom. Please be on lookout for invite.

DEADLINE to sign up will be April 15th @ 3pm

** If your child has not completed their required immunizations or physical examination for school they cannot participate in this activity.**
