The Rotary Club of West Seattle will present an award for community volunteer service based on the Rotary International motto, “Service Above Self.”
AWARD
The recipient will be presented with the award at a Rotary Club meeting towards the end of June (if the recipient can attend). After the selection, the winner will be notified of the date and time of the meeting.
ELIGIBILITY
•
Youth and young adults up to age 30.
•
Service experiences that exemplify “Service Above Self.”
•
Be a member of the West Seattle community (live, work and/or attend school in West Seattle)
NOMINATION FORM INSTRUCTIONS
•
The nomination form must be filled out completely and legibly. The statement of service must be not more than 250 words.
•
You may send a maximum of three attachments to the form (Example: Two reference letters, one newspaper article, etc.) to Tara Luckie at
taraluckie@gmail.com•
The decision of the Rotary selection committee is final.
•
All information will be confidential
HELPFUL HINTS
•
Be specific and concise when possible.
•
Focus on one program that most exemplifies the nominee’s service. Secondarily, describe the other volunteer work as they rank in importance.
•
Assume the committee knows nothing about the nominee and the organization(s) in which the service was performed. Committee members may be unfamiliar with the organization(s), so tell the story as you would to a stranger.
•
Make sure all areas of the nomination form are filled out properly.
Nomination forms must be submitted on or before Sunday, June 2nd.
