This form serves as the official acceptance of your nomination and agreement to appear on the ballot for the Minnesota Excellence in Learning Academy School Board. Only fill out this application if you have already been nominated on THE NOMINATION FORM. If you do not know if you have been nominated, please contact Traci Moore (tmoore@melaschool.org).

YOU MUST ALSO, Email Jossjan Morales (jmorales@melaschool.org) to complete the background check form.

OPTIONAL: If you would like your picture on your candidate bio, please email the image you would like included to Traci Moore (tmoore@melaschool.org).

The Board of Directors must be composed of at least five members who are not related or affiliated with one another, with at least one of each of the following: 1) licensed teachers teaching in the classroom; 2) a parent or legal guardian who is not an employee of Minnesota Excellence in Learning Academy; and 3) a community member who is not an employee or parent of Minnesota Excellence in Learning Academy. Immediate family members of school employees and non-teaching employees are not eligible to serve on the Board.
