 CruCon: Cohort Registration Form
Congratulations! You have prayed, been trained, and are now within weeks of launching your own Conversation Cohort. This form is what we need each Conversation Partner to complete in order for us to get the Convo Packets to you in time for your Series Launch.

Please Note: If you are a part of a multi-church cohort, then only the lead Conversation Partner needs to fill out this form, based on your cohort's registration process.

For CruCon's awareness: If there are any of your Convo Groups that are not all members of the same family (i.e., a mentor or pastor leading a Convo Group instead of a parent) please name how many below. 
Cost breakdown: As we are a nonprofit all, of the following are suggested donations. 

Access to CruCon Series: The cost for this initial Wisdom & Media Project is $400 for an individual church or $200 for churches collaborating on a multi-church initiative.

CruCon materials per Convo Group (unless having participants order their own) = $15 (Conversation Guide, stickers, and bookmarks)

Please total your donation based on whether this is a single church ($400) or multi-church initiative ($200 per church), and $15 x the # of registered participants.
