This is the official registration form.  People filling this form will eventually receive a participation certificate (only if they achieve the goals of the event, i.e. minimum attendance rate, scores in tests, etc.)

By filling this form students acquire the compromise to attend lectures and present tests.  It would not be mandatory to pass all tests and obtaining low scores will not affect academic records.
