In order to participate in the Whitehall-Coplay School District's interscholastic sports and/or extra-curicular activities, the Home Education Supervisor must complete this form EACH week by 12:00 PM every Friday for the duration of the enrollment of the activity and/or sport. Failure to complete this form results in the student not being able to participate in the following week's games. Please note: This form is only intended for teams and clubs affiliated with the Whitehall-Coplay School District.
