By completing and submitting this form, you are indicating your intent to hire me, Kivilcim Ozaner, as your U.S. Agent under CFR Title 14, Part 3, Subpart C. Within three business days of submission, you will receive an email to sign the service agreement and make the annual subscription payment.

Service Fee Schedule:

  • Standard Fee: $79.00/year per Applicant
  • Student Discount: $53.00/year for applicants enrolled in Kivilcim Ozaner’s A&P test prep course

Please note: Completing this form does not designate me as your U.S. Agent. Your designation will only be valid after signing the agreement and making your payment. You may cancel your request before payment is made, but once payment is received, your service will begin.
