 Brookside Charter Extended Care Enrollment Application 2026–2027 School Year.

To enroll your child in Extended Care, please complete the following steps:

1. Complete the Application

Fill out and submit the Extended Care Enrollment Application.

2. Pay Enrollment Fees

  • $25 Registration Fee
  • $35 for the first child
  • $15 for each additional children

Payment Options:

  • Front office with Ms. Dilworth
  • Online at brooksidecharter.org

3. Attend Orientation

Attend an orientation meeting with the Extended Care Director to review the program expectations.

4. Child Care Subsidy Families Only

If you receive childcare assistance (DVN #000184492), please provide:

  • A copy of your approval letter
  • Any required parent co-pay or sliding-scale fee documentation

Important

Enrollment is not complete until all required forms, fees, and documents have been submitted and approved.
