The FinalForms and OHSAA Physical must be completed before practicing or trying out for a sport. FinalForms must be completed by both the parents and the student. When creating a student FinalForms account you CAN NOT use the students school email account. The student MUST use a separate email account. A doctor must complete the OHSAA Physical Form. The physical is valid for one year from the date of exam that your doctor writes on the form. Student School Fees(MUST BE CURRENT PRIOR TO TRYOUTS AND PRACTICE) - The student's fees must be current in order to participate in practice or tryouts.

Athletic Participation Fees(MUST BE PAID PRIOR TO 1ST GAME) - DO NOT PAY PRIOR TO TRYOUTS!! The fees are based on the lunch schedule. If the student'athlete is on regular priced lunch the fee is $90, reduced lunch is $65 and free lunch is $40

Academic Eligibility - The student must pass at least 5 classes during the previous grading period in order to be eligible to start the season. The student must maintain eligible by passing at least 5 classes during the season. Grades are checked at the end of each grading period.

Behavior - We expect all of our athletes to be good role models not just in sports but in school and the community.
