In consideration of my participation as a volunteer with the Church Street CRC, I agree to assume all risks incidental to such participation and, on my own behalf, and on behalf of my or my heirs, executors and administrators, release and forever discharge the released parties listed below of and from all liabilities, claims, actions, damages, costs or expenses of any nature arising out of or in any way connected with my participation in such activity, and further agree to indemnify and hold each of the released parties harmless against any and all such liabilities, claims, actions, damages, costs or expenses, including, but not limited to, all attorney's fees and disbursements. I understand that this release and indemnity agreement includes any claims based on the negligence, action or inaction of any of the above released parties and covers bodily injury (including death) and property damage, whether suffered by me before, during or after such participation. I declare that I am physically fit, have the skill level required, and am able to perform the duties required for this role. I further authorize medical treatment for myself, at my cost, if the need arises. I understand that I, as a volunteer, am participating in such capacity of our own free will and that no insurance coverage is being provided on my behalf by any entity associated with the Church Street CRC. I also understand that I may be required to leave the premises should I exhibit undesirable conduct. I further grant the released parties the right to photograph and/or videotape me and further to use my name, face, likeness, voice and appearance in any form of media including, but not limited to exhibitions, television, publicity, advertising and promotional materials without reservation or limitation. I further release the aforementioned parties from any possible injury to my personal and community reputation, status or image or any other real or imagined damages arising out of or in connection with the aforementioned. I agree that I shall not be compensated in any way for the use of my name, face, likeness, voice and appearance in connection to the aforementioned. The released parties are, however, under no obligation to exercise said rights herein granted.
