This form is to assist manufacturers in determining what documentation is required for Vegan Action to approve an individual ingredient being submitted for Vegan Certification.

This form is required for the following ingredients: all sweeteners, beer, wine, liquor, prebiotics, probiotics, cultures, vitamins and oral minerals, vinegar, juice, bacterial strains, cultured ingredients, and enzymes.

All questions require an answer. Please put NA if the question does not apply.
