Complete this form if you are a local author who is requesting your book be added to the Lisle Library District's (LLD) collection or if you are requesting a friend or family member's book be added to the LLD's collection.

The Library is unable to individually respond to each inquiry due to the large quantity of requests received. All submissions will be reviewed by appropriate LLD staff. In order to streamline workflow, the Library asks for no phone calls or emails regarding Local Author requests.

Note- if you would like to donate a copy of your book, you must first receive approval to do so after completing this form. Staff will be unable to accept books that have not been approved.

If the LLD needs more information and/or your request is accepted, LLD staff will connect with you via the contact information provided on this form.
