By typing my name, I attest to the accuracy and truth of this application. I understand that this is the first step toward adopting from A Place for Us Greyhounds Rescue (APFUGR) and that I can expect follow-up in the form of telephone calls/conferences, emails (be sure and check spam for replies), and home visits (in person or virtual). I may be asked to provide personal references and APFUGR may contact my veterinarian. Submitting and signing this consent gives APFUGR the authority to contact additional persons, if necessary.  Applicants that are living with their parents will be treated as minors requiring parental consent and participation-regardless of their age. I AGREE TO CONTACT APFUGR IF AT ANY POINT, I DECIDE NOT TO PURSUE THIS APPLICATION. (Don't "ghost" the director--she's getting too old to put up with that.)
