Release of Liability
I am the parent or legal guardian of the above-named student and voluntarily consent to their participation in classes, workshops, camps, competitions, and performances with Lake City Highland Dance. I understand that participation in dance activities involves inherent risks of injury.
I hereby release and hold harmless Lake City Highland Dance, its owners, instructors, staff, and affiliates from any and all liability for injury, illness, loss of or damage to personal property arising from participation in these activities, whether known or unknown.
I further understand and agree that I am responsible for any and all damage to studio property, equipment, facilities, or the property of others caused by my student’s actions, whether intentional or accidental. I agree to promptly reimburse Lake City Highland Dance for the cost of repair or replacement of any such damage.
In the event of an injury or medical emergency, I authorize Lake City Highland Dance to obtain necessary medical treatment for my child. I understand that I am responsible for all related medical expenses. I acknowledge that participation is voluntary and that my child may decline to participate in any activity.
