Mondo Bazaar Holiday 2015 - vendor application form
Thanks for your interest in Mondo Bazaar!

Please use this form to enter your application for our upcoming holiday show, to be held on Saturday December 5th, and Sunday December 6th from 10:00am - 4:30pm (each day) at the Gladstone Hotel.

Table fees are $70 for the Melody Bar and $95 for the Ballroom.

The deadline for applications is October 23rd and applicants will be notified of their acceptance by October 30th or sooner.

PLEASE NOTE: In addition to completing the following application form, you are required to email 2 photos of your work to mondobazaar2015@gmail.com

Payment: If you are accepted into the show, you will be asked to send the application fee by e-transfer as the preferred payment method.
