The following survey will assist the Madison SEPAG in developing goals and objectives for the coming year. We hope to provide members with discussions and workshops that match their areas of interest.

All responses will be anonymous and confidential. Survey data will be reviewed by the SEPAG leadership only. Survey results will be shared with SEPAG membership, school leaders and board of education where appropriate.

Please only complete one survey per household. If you have multiple children receiving services you will have the opportunity to indicate below.
