By submitting this form, I fully acknowledge I are aware of the risks, and agree not to hold the Bristol/Mt. Hope Hockey Association, the volunteer coaches or the ice arena liable for the consequences that may occur if we participate in the activity. 

This program is being offered completely free to students enrolled in the Bristol/Warren regional school system, and/or students who intend on going to Mt. Hope High School for grades 9-12. While the clinic is free, we ask that if you enroll in the clinic, you make a strong effort to participate in as many sessions as possible.
