我理解并同意下列入学要件：
1) 我理解，我的孩子（们）如果持有或使用烟酒毒品，不论是在校内或校外，将会被退学和勒令离校，并根据情节轻重，由教会行政人员移交其他惩治机构对其进行处理。
2) 为了增进学习效率，我同意提供我的孩子（们）一个与学校相容的校外环境，尤其是道德方面的教导。
3) 本校的教务及管理人员有职责及职权，对我的孩子（们）的不当行为按照学校规定做必要的纠正处分。
4) 做为学生家长或监护人,我理解,如果在学年当中,未能克尽分配的校务职责,并没有缴交八十元替代费者,我的孩子(们)可能因此无法来校上学。
5) 如学生无法来校上课时，学生家长或监护人应负责在事前向班级老师报备。
6) 凡本校学生，在学年中，应勤勉出席上课。
7) 学费是以每个学期(每一学年包括秋季和春季两个学期)计算，并应在规定注册期间缴交。
8) 认同学校和家庭之间良好的沟通和标准统一的重要性，作为父母或监护人，承诺出席参加本校的家长会以及校方与家长的沟通会。本会的召开，除非是有不可预见的事由或时间上的冲突，应会在初秋举行。
9) 在我的孩子（们）于康福中文学校上课期间，我同意校方的免责声明。并承诺不因我的孩子（们）在学校可能的受伤而针对学校采取诉求，行动以及要求赔偿。同时也不会起诉康郡教会，中文学校及其教职员工。
10) 本人已读过并了解康福中文学校家长手册
11) 我阅读过，也完全同意这份康郡福音教会附属中文学校的入学同意书。

I understand and agree to the following conditions of admission:

1) I understand that if my child possesses or uses alcoholic beverages, illegal drugs or tobacco products at or away from school, he/she may be suspended or dismissed from school or subjected to other disciplinary measures at the discretion of the Church Administration.
2) Desiring my child’s school program to be effective, I agree that I will maintain for my child an environment away from school which is compatible with the school, especially in the area of moral standards.
3) The faculty and administration of the school have the responsibility, as well as the authority, to discipline my child as they judge necessary.
4) Parent/guardian who do not participate in school duties during the school year, nor paying the in lieu fee $80, should understand that their child may not be able to attend the school.
5) Parent/guardian shall bear the full responsibility to notify the teacher of student’s intended absence prior to the beginning of the class.
6) Student shall be diligent in class attendance during the school year.
7) Tuition fee is paid for one semester and is due on the date of registration.
8) In recognition of the critical importance of unity in standards and clear communication between the school and home, we commit ourselves, father and mother, to attend the School PTA meeting to be held early in the fall, unless some unforeseen, unavoidable schedule conflict arises.
9) By enrolling my child/children in CCGC Chinese School, I release the school from all liabilities during the school year. In consideration of my child/children participating in the programs of CCGC Chinese School, I agree on behalf of my child/children to assume all risks of injury of my child/children and to waive all claims, actions, and damages against CCGC Chinese School. I further agree not to sue CCGC Chinese School, its staff, and CCGC.
10) I have read and agree with the Parents Guide of CCGC Chinese School.
11) I have read and agree with the Statement of Agreement of CCGC Chinese School.
