The Locality Wellness Hub Present The March Break 3 on 3 
 Locality Wellness Hub is excited for a FUN one day tournament on Monday March 16th and Tuesday March 17th, as our players make new friends, allow all levels to play together, and have the chance to get on the ice and play the sport we all love!

Please fill out a form for each of your players.

• Open to 5 divisions for players born from U7, U9, U11, U13 & U15
  March 16th - U11, U13 & U15 -  March 17th - U7 & U9
• Individual player registrations are required
 • Players can request up to two friends per team; we try to accommodate all requests (but not guaranteed)
 • NMDMHA - $30/per player (e-transfer to localitywh@gmail.com when registering)
 • Other Players - $40/per player (e-transfer to localitywh@gmail.com when registering)
•  Guaranteed 3 games 
 • All registered players will receive a lunch food voucher, drink, and snack pack
• Team rosters and schedules will be announced before the tournament
  Each team is responsible to provide own coach/parent on the bench
• A guardian for your player must be present for the day
