2025 2026 Student Absence Form

To inform the office of SVCA of an absence including an early pick up, whether current or pre-arranged. One form must be completed for EACH DAY absent. For example, if a student is scheduled to miss an entire week of school, FOUR SEPARATE FORMS must be completed. In the case of families with multiple children enrolled missing the same days, all absent children may be included on the single day's form. Any phone calls, text messages, or emails sent to SVCA Office to inform of absence will also be submitted as a Student Absence Form by a staff member. 


*Email is required to send a copy of your completed Student Absence Form for your records. In the case of an absence reported by phone calls, text messages, or emails sent to SVCA office, staff will include the primary email of parent/guardian provided on the student registration form. This is a safeguard to prevent fraudulent completion of forms to ensure absences are accurately recorded.
