Want to redo a quiz or other redo?  Or follow the alternative quiz correction procedure?  Then complete this form and compete it by the time/day you have specified.  This form needs to be filled out CORRECTLY for this opportunity.

If either way is chosen, that is the way you need to follow through with.  Upon completion of that,  if you would like another redo, conference with me and you may be allowed a 3rd...and last, opportunity.

IF YOU ARE UNABLE TO MAKE THE REDO YOU SCHEDULED, you are to email me ASAP (preferably before the scheduled time) and explain the situation.  Then, at my discretion, you may be allowed to reschedule by filling out this form again.

This can be NO SOONER THAN 2 CALENDAR DAYS  from when you submit this form.  You will need to have time to study and I will need time to create/print a new version of the quiz.   Make sure you do it on a day I am at your school.  The A/B Calendar can be found on the district website.  You are responsible for making sure I'm at your school on that day.
