Dear Driver-Partner,

To enroll and enjoy the extended Chevron Caltex's fuel discount, you will be required to fill up this consent form. Upon getting your consent, we will release your personal information to Chevron.

The submission of personal information would be strictly confidential, and would only be used for the purpose of enrollment into the Chevron Caltex Fuel Discount program.

Starting from 1 July 2021 onwards, all Caltex fuel benefits will be accessible via the CaltexGo app.

On a weekly basis, eligible Driver-Partners who submit this consent form BEFORE Sunday 2359hrs will be activated on CaltexGo app by the following Thursday 0001hrs. After activation, they can proceed to download and register on the CaltexGo app to redeem the fuel benefits.
