External Events/Fundraising Form

In order to ensure that all proposed external events are in keeping with the NFMCPA organizational guidelines, and to comply with regulations pertaining to nonprofit organizations, the NFMCPA requires the following:

1. You must complete and return the External Event Approval Form in order to receive authorization to conduct an event. The NFMCPA will respond to requests no later than 15 days after the receipt of the signed document.

2. The use of the NFMCPA name, logos or other pertinent materials will only be authorized after completion of the External Event Approval Form. The event sponsor is authorized to use the NFMCPA name only in connection with the specified fundraising event, and only until the completion of the event or termination of this agreement (whichever comes first). All usage of the NFMCPA trademark shall bear the trademark symbol (™).

3. Copies of all materials in which the NFMCPA names, logos or other pertinent materials that have been printed in connection to the event must be sent in their entirety to: National Fibromyalgia & Chronic Pain Association, 31 Federal Avenue, Logan, UT 84321. You may also fax the materials to 435-752-3556; or email them to info@FMcpAware.org

4. At the NFMCPA's discretion, Association discounts may be offered on materials that are appropriate for the event. These arrangements, including shipping and handling, will be negotiated for each individual event.
