Small Animal Surrender Form
ATTN: We are only able to take in small animals within Geauga County at this time. We are currently experiencing up to a six month wait for in-county small animal intakes. (Last update 1/6/22)

If you are not a resident of Geauga County and the pet owner, please contact your county's humane society or animal welfare agency for rehoming services.

Please utilize social media, and other rehoming platforms to try and rehome your small animals.

If you have more than one pet you need to rehome, we will need one form PER pet in order to assess every animal individually for intake.
