LIABILITY RELEASE FORM

I hereby acknowledge that I have voluntarily agreed to participate in the Bike or Hike Open Mic event organized by Beach Cities Rock Club (BCRC).

In consideration of being allowed to participate in the event, I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of my participation in the event.

I hereby release, waive, discharge, and covenant not to sue BCRC, its officers, employees, volunteers, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to any injury, disability, death, or loss I may suffer, whether caused by the negligence of BCRC or otherwise, while participating in the event.

I further agree to indemnify and hold harmless BCRC, its officers, employees, volunteers, agents, and representatives from any and all liability, claims, demands, or causes of action arising out of or related to my participation in the event.

I understand that this release of liability covers all claims, whether caused by the negligence of BCRC or any other person or entity.

I certify that I am over the age of 18 and have read and fully understand the terms of this release of liability, or if I am under the age of 18, that my parent or legal guardian has read and fully understands the terms of this release of liability on my behalf.

This release shall be binding upon me, my heirs, legal representatives, and assigns.

Photo & Video Release Form:

I hereby grant permission to Beach Cities Rock Club (BCRC) to use my image and/or likeness in any video, photograph, or audio recording taken by or on behalf of BCRC during my participation of this performance.

I understand that these recordings may be used for promotional or educational purposes, including but not limited to website and social media posts, marketing materials, and presentations. I acknowledge that my child's first name may be included in conjunction with the recordings.

I release and discharge BCRC and its representatives from any and all claims, actions, demands, or liabilities arising out of or in connection with the use of my image and/or likeness in the recordings, including but not limited to any claims for invasion of privacy, defamation, or copyright infringement.

I certify that I am over the age of 18 and have the right to enter into this agreement, or if I am under the age of 18, that my parent or legal guardian has given their consent to this agreement on my behalf.

This release shall be binding upon me, my heirs, legal representatives, and assigns.
