This form collects the information necessary to maintain an accurate PAC membership record and document the experience, expertise, and representation of committee members.

Massachusetts Department of Elementary and Secondary Education Guidance requires PAC membership to include appropriate representatives of business and industry, organized labor, postsecondary education, parents/guardians, students, and registered apprenticeship programs when applicable. PAC membership should also reflect diverse perspectives and the communities served by the school.
