CBCGB Awana 2016-17 Application
Dear parents,

Thank you for your interest in Awana, a part of children's ministry program at CBCGB.  In order to commit your child to the program, you must complete both 1) CBCGB program requirements and 2) Awana registration requirements.

*****CBCGB Children's Program Requirements:*****
1: Annual Child Information Form (2016-17) is to be completed. This is the paper form needs to be filled up and signed for ANY child to attend ANY of the children's program at CBCGB during the specified school year.

2: For the safety of your child at least one parent needs to be at church property and be reachable by cell phone when your child is with any children's program in CBCGB. Parents need to drop off/pick up in classroom. Computer check-in/out is also required.


*****AWANA Requirements:*****
     Duration:   2016-17 (Friday night 09/09/2016- 06/16/2017, except for holidays and during school break).
     Who:         children age 3 (born on or before August 31, 2013) to Grade 5
     When:       7:45-9:45 pm Friday night
     Where:      CBCGB 1st floor

1: Registration
    - Fill out this online form
    - Pay $20 registration fee (includes required handbook), if attending 2 or more times
2: - Be on time (7:45pm) and prepared
    - Kids in K to grade 5 are required to have his/her own Bible
    - Kids in K to grade 5 are required to wear sneakers to participate in games

We welcome all children to our church on Fridays. Our classroom space is limited. Should the classroom reaches max capacity we will provide an alternative program for those arrive after in an overflow room.
