As per Board policy 5250, pupils may be excused from any part of the instruction in health education, family life education, sex education or instruction that includes dissection of animals that the parent(s) or legal guardian(s) of the pupil or the adult pupil finds to be morally, conscientiously or religiously offensive. 

Complete the form below to submit your request to have your child excluded from a health lesson. The form must be submitted at least two days in advance of the scheduled lesson. 

Please be sure and ensure that your child knows that they will be excused from the requested lesson/unit.
