The Banyule BUG Committee has agreed that whenever an incident or accident occurs on
a scheduled B-BUG ride*, an
Incident Report Form should be completed by the Ride Leader as soon as possible after the event, or by another rider who was present.
The term “incident” can be interpreted broadly, ranging from major falls, crashes and collisions which might require an ambulance and paramedic assistance to falls, grazes and cuts that were fixed without assistance.
Equally important is the possible involvement of third parties — e.g. colliding with a pedestrian, scraping or colliding with a vehicle; damage to a non-member's bike, etc. Some incidents may have implications for our insurance.
You may not have all of the information requested at your fingertips. Don't worry - just put down as much as you can remember. If necessary, the report can be amended later as more facts come to hand.

* a scheduled B-BUG ride includes only Lycra and Latte group rides listed on the monthly programs, any country rides organised by B-BUG and travelling from and to home for club rides. Private rides, alone or with friends or family, are not included.
