Mx. Juneteenth: A Black and Queer Liberation Celebration- Performer/Artist Sign Up
Thank you for your interest in performing at Mx. Juneteenth: A Black and Queer Liberation Celebration! We're so excited to celebrate Black and queerness in Cleveland, Saturday June 18, 2022! The following form is meant to be completed by folxs interested in performing on the main stage of the event!  Programming will take place from noon-6pm EST at 4701 Perkins Ave, Cleveland, OH. Please read the following performer/artist agreements before completing.

PERFORMER/ARTIST AGREEMENT:
-Performers and/or Artists are expected to arrive no later than 30 minutes before there set time.

-Mx. Juneteenth planning committee will provide a written agreement outlining the agreed upon honorarium fee to be paid 50% at the time of booking and 50% after the successful completion of the set.
 
-Please complete by Sunday, May 1, 2022. A member from the Mx. Juneteenth leadership team will reach out shortly thereafter.
