Eddins Student Dismissal Change Form: 2025-26
This is to CHANGE the way your child typically goes home. If today will be DIFFERENT from a normal day,  please submit dismissal changes FOR TODAY ONLY on this form. This form will go directly to the office staff and your child's homeroom teacher. All changes for dismissal must be submitted by 2:45 PM.

If you have multiple students, fill out ONE form PER STUDENT!  The teachers will NOT SEE the information if you include a second student in a response.

Please do not send dismissal changes to teachers. Use this form, and teachers will be notified.
