This Form is required for any absence from rehearsal 30 days prior unless a medical or immediate family emergency. This form is due by July 11th for all performances including but not limited to: football games, parades, playoff games, competitions, or state football games.

Forms submitted less than 30 days prior can result in: sitting during third quarter, inability to perform in some capacity, inability to perform at the next competition, and in extreme circumstances removal from group.
