For us to be able to process a surrender request we require detailed  information about your dog to enable us to evaluate how best to support your dog, and to enable us to assess whether we can place your dog in one of our foster families.  Please ensure that you are as honest and thorough as possible when filling out the form.  Any medical, training or behavioural issues or information you provide will enable us to ensure our volunteer foster families are fully prepared, as well as enabling us to put in place any necessary provisions to meet your dog's needs.  If you wish to submit further information - ie a behaviourist report - in support of your surrender request, you can email this to us:  doodlerescueuk@gmail.com.

It is not possible to reclaim a dog once legal ownership of the dog has been transferred to Doodle Rescue and is in our care. We therefore encourage you to ensure you have carefully considered your decision to relinquish legal ownership of your dog to Doodle Rescue. We understand that deciding to give up a dog is not easy and we would encourage you to talk to someone if you are struggling with this decision.

If you have a disability which impacts your ability to complete this form, please ask someone to assist you, and please make us aware of your disability and how it affects you when we confirm receipt of your completed form.

As a small charity, run entirely by volunteers and dependent upon donations and the increases in vet fees and other costs associated with the care and welfare of dogs.   If you are able to make a donation towards your dog’s care costs whilst in foster with us this would be greatly appreciated. You can donate via our website: https://doodlesforrehoming.uk/donate.html or by bank transfer: Account Name: Doodle Rescue Account Number: 39527162 Sort Code: 23-05-80 – please give your dog’s name as a reference.

It is important to note that if you are not the sole legal owner of the dog, anyone else with a legal claim to them must be in agreement with the decision to relinquish legal ownership to Doodle Rescue. It is your responsibility to ensure that anyone else with a legal claim to the dog is made aware and is in agreement with the decision to relinquish legal ownership of the dog, and Doodle Rescue accepts no responsibility for any outcomes should you fail to do so.

Are you the SOLE legal owner of the dog?  If your answer is NO, please explain your relationship to the legal owner and what authority you have to act on their behalf in surrendering the dog, OR please confirm that all parties with a legal claim to the dog are in agreement to the relinquishment of the dog.  Please provide the name and contact details of any other legal owner or of anyone else who has a legal claim to the dog

Once a dog has been accepted for foster and rehoming with Doodle Rescue, it is a requirement that owners sign a transfer of ownership contract which transfers legal ownership to Doodle Rescue. Once a dog has been relinquished to Doodle Rescue and is in our care,  ownership is transferred to Doodle Rescue the dog cannot be reclaimed.      PRIOR TO THE DOG COMING INTO OUR CARE, YOU CAN STOP THE PROCESS AT ANY TIME. THERE IS NO OBLIGATION TO CONTINUE, BUT  IT IS IMPORTANT YOU UNDERSTAND THAT ONCE YOUR DOG HAS BEEN COLLECTED TO COME INTO FOSTER THEY BECOME THE LEGAL PROPERTY OF DOODLE RESCUE AND THERE IS NO GRACE OR COOLING OFF PERIOD.

PLEASE CONFIRM YOUR UNDERSTANDING THAT IN SUBMITTING THE  SURRENDER REQUEST FORM YOU ARE CONFIRMING YOUR WISH TO RELINQUISH OWNERSHIP OF YOUR DOG(S) TO DOODLE RESCUE AND YOU UNDERSTAND,  ONCE WE HAVE ASSESSED YOUR DOG AND HAVE OFFERED A FOSTER PLACE, YOU WILL BE REQUIRED TO SIGN THE TRANSFER OF OWNERSHIP CONTRACT AND THEREAFTER YOU WILL NO LONGER BE THE LEGAL OWNER(S) OF THE DOG.
