F1B - Nepean Therapy Dog Volunteer Application Form (With Dog)
This application is for a dog-handler team volunteer position at Nepean Therapy Dogs.

Once this form is completed, we will send you an email in response to your inquiry shortly.

If your application is successful, you will be invited to a Pre-requisite Suitability Test, where you and your dog are assessed in various aspects of behaviour which are significant in our program. These tests occur on Saturdays, so you will have the opportunity to organise a date with our volunteer intake assessor.
