Diplomas are being ordered for your graduation. To complete the order, the name to be printed on your diploma needs to be confirmed. Please discuss with your parents how you would like your name to appear on your diploma before completing this survey. The name on your diploma will also be listed in the official graduation program; however, the name to be read when you walk across the stage does not have to match the name printed on your diploma.

Check the spelling of your name (first, middle and last) when submitting. If you have an accent that needs to be included on your name, please email Ms. Willis before the deadline.

PLEASE COMPLETE THIS FORM BY Thursday, December 16, 2021. If this form is not completed by that date, your name will appear on your diploma as it appears in PowerSchool. Any late changes to diploma names will require an additional cost to process.
