DPMS Schedule Corrections 2025-26
This form is to be filled out by parents of a student who is: missing a class, needs an intervention class, has classes that are cross-teamed or another Skyward error. 

As stated on your course registration card, class change requests and team requests were due by April 15th. We build our master schedule based those requests. If you feel that there is an error on your schedule or other concerns, we will happy look into it and get back to you. 

School counselors will work on the correction list in order that we receive them. Thank you for your patience and understanding. 

Any changes made to your schedule may effect other classes.  All changes are final.
