Dance Class Waiver & Release of Liability
By registering for and/or participating in this dance class, I acknowledge and agree to the following:

I understand that dance involves physical activity and carries inherent risks of injury, including but not limited to falls, strains, sprains, or other physical harm.

I certify that I am physically fit to participate and that I have consulted with a physician if I have any medical conditions or concerns.

I voluntarily assume full responsibility for any risks of injury or loss that may arise during or after my participation.

I hereby release, indemnify, and hold harmless the instructor, staff, and hosting organization from any and all claims, liabilities, or damages arising from my participation in this class.

By attending this class, I confirm that I have read, understood, and voluntarily agree to the terms of this waiver.
