Please submit this form for each child for each Distance Learning Day. Submitting this form acknowledges your child is participating in the required Distance Learning activities for the day.

If you have more than one child enrolled in CCSD21, please complete one form per child as this constitutes a record of attendance and evidence of engagement in learning activities.  Please be sure to complete the form for the school your child attends because attendance will be monitored at the school level by the principal.

If you are unable to complete the form or do not have internet access, a parent/guardian can submit a paper attendance form when the students return to school.
