 Have you  been convicted of a misdemeanor or a felony in the last seven years? *
 I understand and agree that my volunteer service with 4-H Camp shall be on an “at-will” basis, meaning that either I or 4-H Camp may terminate this relationship at any time, for any reason or no reason, and with or without notice, without incurring any obligation or liability, that this agreement supercedes any and all prior agreements or representations made between me and 4-H Camp, and that this agreement may only be modified by a writing that is signed by the Director and that specifically refers to this agreement. *
 I understand that the misrepresentation or omission of information requested is just cause for immediate release as a volunteer from 4-H Camp. Upon satisfactory completion of this application and completion of the screening process, I will be notified of my acceptance/rejection for this position. By clicking the "Done" button, I understand that the above information may be verified by contacting persons or organizations named in this application, and I hereby release from liability any person or organization that provides information concerning me to the representatives of the Rutgers Cooperative Extension 4-H Youth Development Program.
