        1:30 – 3:00 pm – skill development
        6:00 – 7:30 pm - scrimmage
Sr. Girls & Boys 10:30-12:30 am – skill development
        3:00 – 4:30 pm – skill development
        7:30- 9:00 pm - scrimmage

Parental Informed Consent Agreement (for participants under 18 years of age)  We, the undersigned, hereby acknowledge that we have been provided with the description of the activity and we wish our child to participate in the Volleyball Skills Camp to be held at the Lloydminster Comprehensive High School, August 18-20, 2025.  We recognize that this activity could cause some types of injuries, minor or serious, which may result from one’s actions or interactions with others.  A few examples of the type of injury which one is at risk of having occur while engaged in this activity are: Muscle pulls and strains, scrapes and bruises, head injuries.  We understand that the RULES AND REGULATIONS for the sport and activities as outlined by the Board of Education are designed for safety and protection of participants, and hereby undertake to have our child abide by these rules and regulations.  We understand that certain activities require a minimum level of fitness and health (physical, mental and emotional) and that each person has a different capacity for participation in these activities. We undertake to make our own provisions for student accident, injury, and liability insurance.  We also hereby warrant that any known medical or medication needs of our child will be clearly indicated in writing on this form.  We agree that the Lloydminster Public School Division Board of Education, its employees, servants or agents shall not be liable for any injury to our child or loss or damage to any personal property arising from, or in any way resulting from participation in these activities, unless such injury, loss or damage is caused by the sole negligence of the School Division or its employees, servants or agents while acting within the scope of their duties.  We declare having read and understood the above INFORMED CONSENT AGREEMENT in its entirety and hereby consent to participate acknowledging the foregoing. *

My son/daughter will commit to attending from August 18 to August 20.  Registration fee of $175 will be paid at the first day of camp.  All cheques will be made out to LPSD. *
