CCHS Educational Model Change - 2nd Semester
Please complete this form if you are interested in changing your child's educational model for the 2nd semester. The form must be completed by December 9th. Please note that changes WILL result in a schedule change which will go into effect on January 5th. If you have multiple students at Cypress, and you want to change their mode, you will need to fill out a form for each child.
