What province is your Condominium located? *
We have provided a brief guide as to which types of Condominiums would be able to use our online service. For further assistance, contact your property manager and/or legal advisors. Please check off the province that applies to your Condominium.
Required
Your Name and position with the Condominium *
Your Email *
We will be emailing this report to the email address you provide. Please ensure that this is the proper email address. You can provide up to 4 email addresses and we will email the completed Reserve Fund Study to all email addresses.
Your Contact Phone Number(s) *
Payment for the full report ($997.50 including GST) will be required before the report is carried out. We will contact you to obtain and process credit card information.
Date to be used on the report *
Please use Month_Day_Year format (e.g., June 15, 2009). This is important as the 25 year Cashflow Projections will start from the year you indicate. Please refer to our sample Reserve Fund Study (RFS) to see the 25 year Cashflow Projections. Note: RFS for Ontario will use a 30 year timeline; all other provinces will use a 25 year timeline.
Building Name *
This is the name used on the study to represent your building.
Condominium Plan Number *
Commonly referred to as your plan number, strata number, etc.
How many units are in your Condominium? *
Site Address - Street, City and Province *
Please provide us with full site address of your building including a unit number for a site representative.
Site Address - Postal Code *
Financial Information - Current Total Balance in your Reserve Fund *
Financial Information - Rate of Return on Investments *
What is the average rate of return on the investments in your Reserve Fund?
Financial Information - What inflation rate should we use? *
Please indicate the inflation rate you would like us to use in this report.
Building Age *
If you are filling out this form in 2009 and your building was built in 1989, then the age of the building will be 20 years (2009 - 1989 = 20 years). If your building was constructed over a period of several years, please estimate the age.
What are your building's Unit Factors? *
A Condominium is broken down into 10,000 units. If you are in a 10 unit building and all the units are equal, then the unit factor would be 1000 for each unit (i.e., 10,000 / 10 units = 1000). This may not be case with your Condominium as some Condominiums have different square footage in their units and therefore have a different unit factor. The unit factor is generally used to divide all costs of the Condominium. If your Condominium divides the expenses evenly between all units regardless of unit factors, please let us know the common unit factor or simply say something along the line of "Our Condo divides all costs evenly between the units".
