Muddyum® Art Commission Form
This form is necessary in order to book a consultation with Muddyum®. This will help better assess a quote. Once form is submitted you will hear back within 5-7 business days.

After the consultation, once the quote is agreed upon you will be required to sign the contract and pay a 100% downpayment in order to reserve my time to make a piece for you. (This does not include shipping.) Minimum orders are $300.
